I've got orthotics and boots to fit them are on the way (being made). I simply went to the wing surgeon, he said I had very flat feet, referred me to a podiatrist. Podiatrist casted me for orthotics, I picked them up & they don't fit well in my boots so I got a chit to go to the QM who sent me to a shoe store which sold boots accomodating orthotics &c.
Since none of these boots fit me (I would have had to cut the orthotic down to fit it in one shoe, and then the others wouldn't fit, plus only one was almost comfortable) I went back to the QM and they sent me to a shoemaker. Thus, I have handmade combats, parade boots, and oxfords.
And yes, the military paid for it all, minus the $140 running shoes I had to buy to accomodate my orthotics. Also, the custom made combats look exactly the same as the Mk3 combats, if anyone should care.
