I had my medical interview roughly two weeks ago, and just received the forms to request more information from my family physician. After a quick review I have noticed that there is an error on one of the forms in the spelling of a medical condition, actually not just spelling but an incorrect word i.e. "Jake" instead of "Jacobs". How should this be corrected. Should the physician just put in the correct spelling of the issue or should I request a new form?
I do apologize if this has already been answered, but unfortunately I am in a bit of a rush to get these filled out by my physician, and to get this typed, and have not performed a search to see if this has been asked and answered before. If it has please provide where I can get the answer.
