Not to take this off topic, but suppose there were adequate and appropriate rules in place, but they were not being obeyed?

Accidents don’t always happen because rules are ineffective or ignored. If you look at the following picture, you’llhabe a glimpse of how aviation incidents causes are classified. Person centric deviations are extremely rare. Mission centric deviation are fairly rare. Insufficient training and regulations do happen but not to an extent that makes a situation dangerous. Human errors however are the vast majority of issues.
